Crescent Community Health Center serves anyone needing a medical and/or dental home. We can serve people of any age, from any region, with any almost any type of insurance or no insurance at all. This includes Iowa Health & Wellness, Medicaid, Medicare, Hawk-I and numerous private insurances. Our focus is providing primary care to patients and providing referrals to community providers for specialty care. We continue to accept new patients on a daily basis.

Don’t know how to return calls or answer their phone, can leave messages and call repeatedly for days before there is any response.
I couldn’t believe how friendly everyone was. Dr Rachel Ploessl was amazing.
Can’t get through to make an appointment on the phone. You usually get the automated system and even when I leave a message for an appointment no one calls me back.
You have a better chance of getting struck by lightning than talking to a human on the phone. This place is terrible!

The absolute WORST service ever. I have been a RN and was shocked at how this clinic manages to stay in business. Dr. Woods is an excellent physician however.
I felt so touched to find a place that helped so many people who cannot afford to have medical or dental care. They do so much for the surrounding communities as well. We are in Wisconsin and are grateful they are there!
