Fairview Community Health Center is a federally qualified health center that provides primary care services to include – prenatal, pediatrics, family practice and adult services. We request a $25 copay and accept all insurance and Medicare. We will adjust fees on a sliding scale based on income but will also not refuse services for someone who does not have the ability to pay.

Had bad experience so stopped going there but since I started seeing Nicole Wise it’s been like going to visit a friend instead of just going to visit the doctor. Still don’t know why she is no longer there.
